Ankle views are standard plain X‑ray projections used to evaluate the ankle joint and surrounding bony structures. In clinical practice, they help rule out fractures, guide reduction after injury, and monitor healing or postsurgical alignment. The most common projections are anteroposterior (AP), lateral, and sometimes mortise or oblique views, each highlighting different aspects of the ankle mortise, joint spaces, and cortical continuity. These studies are typically ordered after acute trauma, for persistent pain, or during outpatient follow‑up to ensure proper bone healing and joint stability.

Standard Projections and Patient Positioning

Anteroposterior (AP) View

For the AP ankle view, the patient sits or lies supine with the ankle in slight dorsiflexion to open the joint. The beam is centered midway between the medial and lateral malleoli, and the foot should be aligned so the plantar surface is perpendicular to the image receptor. Proper positioning demonstrates the ankle mortise evenly, without significant overlap of the tibia and fibula, and shows the talus seated squarely within the mortise.

Lateral View

In the lateral projection, the patient lies supine with the knee flexed and the foot positioned so the plantar surface is against the image receptor. The central ray is directed perpendicular to the lateral aspect of the ankle. This view best shows the posterior lip of the distal tibia, the curvature of the posterior tibiotalar joint, and evaluates alignment between the talus, tibia, and fibula. It is especially useful for detecting talar tilt and posterior malleolar involvement.

Mortise and Oblique Projections

The mortise view is obtained by internally rotating the ankle approximately 15 to 20 degrees, which aligns the medial and lateral mounds of the talus with the tibial plafond to create an open, symmetrical joint space. This projection is favored for assessing joint congruity and subtle widening that may indicate ligament injury or subtle displacement. Some clinicians also obtain an oblique projection to evaluate the syndesmosis and lateral malleolus from an angled plane.

What Clinicians Look for on Ankle Views

When interpreting ankle views, the primary goals are to exclude fracture, confirm proper joint alignment, and follow healing after treatment. Key anatomic relationships, such as the clear space between the talus and malleoli and the smooth contour of the articular surface, are assessed. Particular attention is given to areas prone to injury, including the distal fibula, medial malleolus, posterior lip of the tibia, and the talar dome. Subtle cortical irregularities, fat‑stranding, or joint widening can prompt further imaging with CT or MRI when clinical suspicion remains high despite normal radiographs.

Limitations and Complementary Imaging

While ankle views are widely available and quick to perform, they have limitations. Plain films may not detect small hairline fractures, occult injuries, or purely soft‑tissue damage. In high‑energy trauma or when clinical findings do not align with radiographic results, advanced imaging such as CT or MRI is often used. CT offers superior bony detail and is particularly helpful for surgical planning, while MRI is more sensitive for ligament tears, bone marrow edema, and early osteochondral injury. Understanding these strengths and limits helps guide appropriate next steps in care.

Practical Considerations and Safety

  • Always confirm patient identity and verify that the correct limb is imaged.
  • Use appropriate shielding for reproductive organs and thyroid region when indicated.
  • Minimize repeat exams by checking positioning and beam centering before exposure.
  • Document technical factors such as kVp, mAs, and any rotation or tilt that might affect interpretation.
  • Communicate clearly with the radiology team to ensure appropriate projections are performed and prioritized based on clinical urgency.

How Ankle Views Fit Into Overall Ankle Assessment

Ankle views are one component of a comprehensive evaluation that includes history, physical examination, and, when needed, advanced imaging. They provide a baseline for acute injury management, support decisions about weight‑bearing and immobilization, and aid in postoperative or rehabilitation follow‑up. When interpreted in combination with clinical findings, they remain a reliable, efficient tool for guiding safe and effective treatment plans.
